Actually this one may be pretty difficult (it's a cult Japanese classic), so i'll add some clues. The director is better known in the West for a horror film where a Widower runs a TV casting session to find a new wife. Also the image is from a scene where a woman gives birth to an adult man

It's one of his films, guess this one's more famous in Japan. It's pretty screwed up, there's a lactating granny in it who bottles her milk.
A mate recommended it to me for a date, completely stitched me up. Just sat next to the girl thinking wtf is this?!
